No information shall be collected without patients written or implied consent. Any Information collected will be treated in a manner that ensures a patients protection of privacy and confidentiality. No information that identifies a patient will be released to any third parties without express consent. Lakeside Sports Medicine Centre will take all possible steps to ensure the protection of your privacy, however there may be circumstances where a practitioner will be obliged to release details of a patient’s personal health without their express consent – this would only be the case if the situation was an extreme emergency, or if legally compelled to do so. All practitioners at Lakeside Sports Medicine Centre operate a fully computerised medical records system; of which, all our practitioners and administration staff have individual passwords to allow access. Our Information is backed up daily and passwords changed regularly. All administrative staff that have access to your personal health information have all signed confidentiality agreements.Lakeside Sports Medicine respects the right for all patients to have access to their medical records.
